• Proof of identity can be an authentic, current U.S. passport or passport card, or an original or certified copy of a birth certificate  with a raised or embossed seal.

• Proof of Social Security number can be a Social Security card with current legal name on it 

• Two proofs of current, physical PA address: current, unexpired PA license or ID, auto insurance card, utility bill with the same name and address or PA vehicle registration.

• If necessary, a certified marriage certificate, court order, or divorce decree from your county's family court may be used as proof of all legal name changes.
